    Evaluating the Improvement of Quality of Life in Rural Areas

    The research starts from the necessity to create specific tools for evaluating the impacts of rural development policies on fragile areas. The study is motivated by the need for developing an appropriate evaluation method that leads to gather meaningful information for a broader understanding of the quality of life in rural areas, including the subjective well-being’s dimensions and its determinants and feeds the policy designs on this specific domain. The multidimensional nature of quality of life is a main challenge in terms of evaluation. Indeed, within the Rural Development Programmes 2007-2013, the enhancement of the quality of life in rural areas is one of the major strategic objectives to be addressed by a menu of measures. Selections of some current literature on the multidimensional nature of quality of life have been used as conceptual basis for analysing the extent to which the European evaluation framework for rural development programmes (EC 1999, 2006, 2010) - based on the intervention logic model, the use of economic indicators and evaluative questions - is able to capture the relevant dimensions of well-being rural people’s lives. A part of the research is based on the analysis of ex-post evaluations carried out in Italy. The evaluations are expected to assess the improvement of quality of life in rural areas as effect of programmes’ implementation. The paper provides two different experiences of quantification of quality of life in rural area: a synthetic measure of marginality as a proxy of quality of life indicators (in Piedmont) and a synthetic index of quality of life (in Emilia Romagna). The paper analyses the financial allocations from the regional, rural development and agricultural policies of the European Union in order to assess their territorial coordination and synergies with the objective of territorial cohesion. Regression analysis is used to uncover the link between funds and territorial disadvantage for the 1994–2013 period. The analysis reveals that both coordination and compatibility with territorial cohesion have not always improved in response to major policy reforms. The territorial ‘vocation’ of overall community spending is weakly linked to its distribution among different policies, but it crucially depends upon appropriate ‘place-based’ allocation mechanisms

    Monitoring environmental and climate goals for European agriculture: User perspectives on the optimization of the Copernicus evolution offer

    Abstract A vicious cycle exists between agricultural production and climate change, where agriculture is both a driver and a victim of the changing climate. While new and ambitious environmental and climate change-oriented goals are being introduced in Europe, the monitoring of these objectives is often jeopardized by a lack of technological means and a reliance on heavy administrative procedures. In particular, remote sensing technologies have the potential to significantly improve the monitoring of such goals but the characteristics of such missions should take into consideration the needs of users to guarantee return on investments and effective policy implementation. This study aims at identifying gaps in the current offer of Copernicus products for the monitoring of the agricultural sector through the elicitation of stakeholder requirements. The methodology is applied to the case study of Italy while the approach is scalable at European level. The elicitation process associates user needs to the European and national legislative framework to create a policy-oriented demand of Copernicus Earth Observation services. Results show the limitations faced by environmental managers in relation to the use of Remote Sensing technologies and the shortcomings associated with a purely technology driven approach to the development of satellite missions. Through the introduction of this flexible and user centred approach instead, this paper provides a clear overview of agro-environmental user requirements and represents the basis for the definition of an integrated agricultural service

    Synergies and conflicts between EU policies and the objective of territorial cohesion

    The paper looks at the overall structure of the European Union’s regional, agricultural and rural development policies in order to assess their coordination and synergies at the territorial level and their degree of compatibility with the objective of territorial cohesion. The regression analysis - covering the 20-year period 1994-2013, and approximately 90% of total Community expenditure - reveals that the compatibility of the various areas of Community policy in terms of the objectives of territorial cohesion has not progressed in a linear fashion over time. Shifting resources in the Community budget from one policy area to another does not, by itself, appear capable of guaranteeing virtuous paths in terms of territorial cohesion.     Bronchiolitis. Analysis of 10 consecutive epidemic seasons

    Bronchiolitis is the leading cause of hospitalization in infants under 12 months. Our aims were to analyze epidemiological characteristics of infants with bronchiolitis over 10 consecutive seasons and to evaluate whether there are any clinical differences between infants hospitalized for bronchiolitis during epidemic peak months and infants in non-peak months. We enrolled consecutive enrolled 723 previously healthy term infants hospitalized at the Paediatric Emergency Department, "Sapienza" University of Rome over the period 2004-2014. Fourteen respiratory viruses were detected from nasopharyngeal aspirates by molecular methods. Clinical and demographic data were extracted from clinical charts. Viruses were detected in 351 infants (48.5%): RSV in 234 (32.4%), RV in 44 (6.1%), hBoV in 11 (1.5%), hMPV in 12 (1.6%), co-infections in 39 (5.4%), and other viruses in 11 (1.5%). Analyzing the 10 epidemic seasons, we found higher incidence for bronchiolitis every 4 years with a peak during the months December-January. Infants hospitalized during peak months had lower family history for asthma (P = 0.003), more smoking mothers during pregnancy (P = 0.036), were slightly higher breastfed (0.056), had lower number of blood eosinophils (P = 0.015) and had a higher clinical severity score (P = 0.017). RSV was detected mostly during peak months, while RV was equally distributed during the seasons. We found some variations in bronchiolitis incidence during epidemics, and discriminative characteristics in infants hospitalized for bronchiolitis during peak months and in non-peak months, that might reflect two different populations of children.     Non-24-hour sleep-wake rhythm disorder and melatonin secretion impairment in a patient with pineal cyst

    We report the case of a 14-year-old girl with a wide non-compressive pineal cyst, associated with the inability to control her sleep-wake schedule. Actigraphic monitoring showed a 24-hour free-running disorder (tau 26.96 hours). A 24-hour serum melatonin curve assay, with concomitant video-polysomnographic and body-core temperature monitoring, was performed. Melatonin curve showed a blunted nocturnal peak, lower total quantity of melatonin, and prolonged melatonin secretion in the morning, with normal temperature profile and sleep parameters. Treatment with melatonin up to 14 mg at bedtime was initiated with complete realignment of the sleep-wake rhythm (tau 23.93 hours). The role of the pineal cyst in the aforementioned alteration of melatonin secretion and free-running disorder remains controversial, but our case supports the utility of monitoring sleep/wake, temperature, and melatonin rhythms in the diagnostic work-up of pineal cysts associated with free-running disorder

    Nasal Microbiota in RSV Bronchiolitis

    Respiratory Syncytial Virus (RSV) is the leading cause of bronchiolitis, and the severity may be influenced by the bacterial ecosystem. Our aim was to analyze the nasal microbiota from 48 infants affected by bronchiolitis from RSV virus and 28 infants with bronchiolitis but negative for the virus. Results showed a significantly lower biodiversity in the RSV-positive group with respect to the RSV-negative group, a specific microbial profile associated with the RSV-positive group different from that observed in the negative group, and significant modifications in the relative abundance of taxa in the RSV-positive group, as well as in the RSV-A group, with respect to the negative group. Furthermore, microbial network analyses evidenced, in all studied groups, the presence of two predominant sub-networks characterized by peculiar inter- and intra-group correlation patterns as well as a general loss of connectivity among microbes in the RSV-positive group, particularly in the RSV-A group. Our results indicated that infants with more severe bronchiolitis disease, caused by RSV-A infection, present significant perturbations of both the nasal microbiota structure and the microbial relationships.     Differential Toll like receptor expression in cystic fibrosis patients' airways during rhinovirus infection

    Objectives: Since an inappropriate and sustained activation of TLRs may contribute to a chronic inflammatory response resulting in detrimental effects in cystic fibrosis (CF) patients, we sought to examine whether HRV infection might alter the respiratory expression of TLRs according to the microbiological status of CF patients. Methods: Respiratory samples were collected from the respiratory tract of CF patients (n = 294) over a period of 12 months. In addition to the usual microbiological investigation, HRV-RNA detection and typing were performed by RT-PCR and sequencing. HRV viral load and TLRs levels were measured by RT-Real Time PCR. Results: HRV-RNA was detected in 80 out of 515 respiratory samples (15.5%) with a similar rate in all age groups (0–10 years, 11–24 years, ≄ 25 years). Patients infected with different HRV A, B and C species exhibited higher levels of TLR2, TLR4 and TLR8 as compared to HRV negative patients. Moreover, the expression level of TLR2, TLR4 and TLR8 correlated with high level of HRV viral load. HRV positive patients co-colonized by Staphylococcus aureus or Pseudomonas aeruginosa showed also enhanced amounts of TLR2 and TLR2/4-mRNAs expression respectively. In the case of presence of both bacteria, TLR2, TLR4, TLR8 and TLR9 levels are elevated in positive HRV patients. Conclusions: TLRs, especially TLR2 and TLR4, increased in HRV positive CF individuals and varies according to the presence of S. aureus, P. aeruginosa and both bacteria
